Parent Chaperones for Field Trips

Logos requires parent chaperones to complete the VIM (Volunteering in Medford Schools) background check before attending a field trip as a chaperone. Please fill out this form if you plan on attending field trips with your student this year. Be sure to select Logos as the school you plan to volunteer at.
